Establishing Your Goals & Needs

Each referral will be scheduled for an assessment meeting to determine if we can help meet the needs of the individual. That determination is based on a comprehensive review of the medical, psychiatric, physical, emotional, social and cognitive factors. We strive to build a relationship with each individual and their circle of support to determine whether the request for services is the best available plan. 

 

Tailored Support Plans

An Individual support plan is created by our team that includes historical data and the goals for supported living or independent living services. This documents guides our team in working with each individual. Each person’s plan will be specific to their needs and is updated as things change.

Helpful Housing Support

We help our client’s maintain their property by assisting with light housekeeping support, maintaining positive landlord/tenant relationships, and can help completing housing application if the individual needs help finding a new home.

Total Care Beyond the Basics

We also assist with the coordinating medical appointments as well as provide medication reminders. Our caregivers are familiar with local community activities and can help with food planning and preparation. Our services includes establishing a  transportation plan to help meet the needs to and from various appointments.  Part of our services includes assistance with accessing generic services in the community, such as In Home Support Services, Section 8, Day Programs, Work Programs, Medical and Mental Health referrals, and Transportation Services.
